Diagnose exists to understand the nature of the challenge, what is preventing the organisation from achieving the agreed outcome, before any strategic response is constructed or actions are designed. It tests the Investment Case from Remake: Align against deeper evidence: validating cost reality, evidencing capability gaps, surfacing dependencies and second-order effects, and producing a clear definition of the actual problem to be solved. Diagnosis is where a remaking stops being a matter of opinion.

## What Diagnose protects against {#protects data-toc="What it protects against"}

The temptation to treat symptoms as causes, to validate convenient narratives, and to produce generic recommendations dressed up as analysis. Many organisations operate on explanations that feel plausible but have never been tested. There is also a subtler failure: diagnosis as confirmation. An organisation sometimes wants diagnosis to validate a course of action it has already chosen. A diagnosis that has been allowed to do that is not a diagnosis; it is a flattery exercise.

## What diagnosis really means {#in-practice data-toc="In practice"}

Four characteristics distinguish diagnosis from analysis-as-activity. It is hypothesis-driven: explicit competing explanations, each tested against evidence, with the aim of disconfirming rather than confirming — a hypothesis that has merely accumulated supporting examples has not been tested. It is multi-source: data, structured interviews, capability assessment, observation of how decisions are actually made, and historical context, triangulated; no single source is sufficient. It is willing to surface uncomfortable findings: that prior investments are not delivering, that pet projects are unsupported by evidence, or that capability gaps sit closer to the executive team than expected. And it distinguishes symptoms from causes: stalled pilots, low adoption, and governance friction are usually consequences of deeper issues, and the diagnosis is complete only when the work has reached the level at which intervention would actually matter.

## How Diagnose runs {#how-it-runs data-toc="How it runs"}

Diagnose begins when the problem frame from Align is stable, constraints are explicit, the scope is tight enough to investigate properly, and leadership is ready for evidence that may disturb prior assumptions. The work is structured around competing hypotheses, with evidence gathered and tested through deliberate disconfirmation. Synthesis happens iteratively, not at the end: the diagnosis tightens as evidence accumulates. Solution roadmaps, vendor recommendations, and target operating models are deliberately excluded; introduced inside Diagnose, they conflate symptom with cause and pre-empt the strategic choice that belongs at Remake: Advise.

## The output {#output data-toc="The output"}

Diagnose contributes the diagnosis to the Transformation Plan: a coherent, evidence-based statement of the core challenge, root causes distinguished from symptoms, explicit constraints and trade-offs, the leverage points where action would matter most, and the disconfirmed hypotheses — what we now know the problem is not. The diagnosis is accepted by the sponsors as the basis for choice, even where it disturbs prior commitments.
